Save time by using our secure Esign forms. Do the initial intake paperwork & screening on your phone, tablet, or computer so that you are set to be contacted and scheduled by our staff the next business day. This is available 24/7.

In addition to completing the intake packet, please complete a screening tool for the service you request. The esign forms are below.

A staff member will take some basic info and send you a link to the patient portal. The link is available during business hours and when we can answer the phone. You must be registered first.

#3. OPTION: DOWNLOAD OUR PDF’S, FILL OUT THE FORMS, AND SCAN, fAX, OR EMAIL THEM BACK TO US.

FYI:
If you are new to services here, we will do a CCA or comprehensive clinical assessment prior to all services including Counseling, Psychiatric Services, and Psychological Services. If you are actively involved with clinical services elsewhere and are a psychiatry referral, we will need your clinical home to send us a recent CCA. If you are a referral for a psychological assessment, we will need to do a CCA and/or update to gather needed information and releases prior to conducting psychological testing and providing a cost estimate.
